Patient-Centered Medical Home (PCMH)

Achieving the designation of a Patient-Centered Medical Home (PCMH) organization can be a challenging task for any organization. The patient-centered process includes the development of partnerships between your practice, individual patients, other providers patients’ utilize and patients’ families, when appropriate. Care is coordinated utilizing information technology, health information exchange (HIE) and other means to make certain the patient receives the right care at the right time in the right place by the right provider for improved patient outcomes. The HealthPOINT team can assist you in determining eligibility, developing a plan and timeline to meet the various elements of recognition, workflow and system redesign, and tracking your progress.
