“Located throughout rural and frontier areas of South Dakota, Prairie Community Health consists of five federally qualified health centers.  Regarding our recent electronic health record (EHR) system adoption, we began the practice management module on our EHR in June of 2010.  In 2011, we went live with the clinical module of our EHR.  Shortly after beginning work with  HealthPOINT in January of 2011 , and with their assistance, we created an EHR Adoption team to place an organizational focus on this important initiative. The EHR Adoption Team consists of our Director of Operations, one of our providers, a member from HealthPOINT and our Administrative Assistant.

The first course of action was the resurrection of our Super User Group by the EHR Adoption team.  The Super User group is comprised of 4 nurses, 1 provider, and 1 receptionist all from our various sites.  The goal of the Super User group is to discuss the EHR issues that the clinics face on a day to day basis.  This information is then be brought to the EHR Adoption team to review the issues and find solutions. We have found that many issues relate to workflows.

One of the first issues that we chose to tackle was our fax server workflow.  Since “Going Live”with our EHR, we have had issues with incorrect labeling, critical values being overlooked, and faxes not getting to their proper destination within the EHR.  To prepare the Super User group for the workflow assessment process, HealthPOINT provided a training session to the Super User group teaching them how to gather information and document the current workflow. Each clinic was then asked to document their current workflow and bring it to the EHR Adoption team.  Kevin from HealthPOINT and I then began to analyze the workflows and created a uniform workflow that all sites would use.  We mapped out the workflows using a “Swim lane style” flow-chart, marking the responsibilities when receiving and sending faxes using our EHR program.  This process was rolled out to the Super User group for final approval as of July 25, 2012.

We are now to the point of creating a training document that the employees can access to troubleshoot through any issues with the fax server.  This document will be user friendly and easily accessible from any computer on the network.  When we roll this process out to the employees, I feel confident that it will help define the patient’s chart, make it easier to review any notes in that chart, as well as have a more timely turn around in dealing with our documents such as labs, consults, etc.

We owe special thanks to our incredible staff for responding to the challenges associated with our EHR adoption and in achieving Meaningful Use.  And we owe a special thank you to Kevin Atkins of HealthPOINT for being an integral part of our EHR Adoption team and helping us overcome those issues that inevitably arise.  It’s been a tremendous partnership.”
